Side effects

Bloating and wind on a GLP-1

A tight, full belly hours after a modest meal is one of the more uncomfortable surprises of a GLP-1. Bloating and wind are listed side effects, and they share a cause with most of the others: food moving through more slowly than it used to.

The short answer

Bloating (abdominal distension) and wind (flatulence) are listed GLP-1 side effects. In the weight-management trials, bloating was reported by 7% on semaglutide 2.4 mg against 5% on placebo, up to 4% on tirzepatide against 2%, 4.5% on liraglutide 3 mg against 3%, and up to 9% on orforglipron against 3%; wind by 3–6% against 2–4%.1234 Smaller meals, eating slowly and increasing fibre gradually tend to help. Severe pain with a swollen abdomen, vomiting or being unable to pass wind needs urgent care.

Why it happens

The medicines slow stomach emptying, and food that sits longer ferments more.12 Swallowed air from eating quickly, fizzy drinks, and a sudden increase in fibre all add to it. Constipation, common on these medicines, makes bloating worse — see constipation.

What tends to help

  • Smaller, more frequent meals, as the 2025 joint advisory suggests for gastrointestinal side effects generally.5
  • Eating slowly, and stopping at the first sign of fullness.
  • Increasing fibre gradually, with fluid, rather than all at once.
  • Going easy on fizzy drinks and very fatty meals.
  • Moving — a short walk after meals helps many people.

When to contact your prescriber

Keeping a record of it

Log bloating with a severity and a time, on the same timeline as your doses, meals and fluids. After a few weeks the record shows whether it follows dose increases, particular foods or the days after an injection — and the Visit Summary puts that on one page for your prescriber.

Cafeity dose screen: an estimated medication-level curve labelled "Estimate based on published half-life. Not a measurement", the last dose, the injection site (stomach, lower left), a countdown to the next scheduled dose and a side-effect check-in.
A side-effect check-in under every dose.
Cafeity "Your week, understood" screen: Ora explains that everything is calculated from your own log, offers questions such as "When are my symptoms worst?", and shows the week's protein, energy, water, weight trend, symptom days and a note that protein was 80 g under target.
Ask when your symptoms are worst.

Common questions

Why am I so bloated on a GLP-1?
Slower stomach emptying means food sits longer; constipation, eating quickly and fizzy drinks add to it. Bloating is a listed reaction on the labels.14
Does bloating go away?
Like other gastrointestinal reactions it is usually worst during dose increases and eases over time; managing constipation often helps too.2
